Feedback, compliments, concerns and complaints

Supporting you to share feedback about your experiences

Feedback, compliments, concerns and complaints

Understanding how patients experience our care and services is essential, and we welcome your feedback.

Read below how to provide feedback by phone, via email, in person, in writing or via the online feedback form.

How to share your feedback

You can lodge your compliment/complaint or feedback by various methods.

  • Speak with a staff member – wherever possible, provide verbal feedback directly to the staff member/s involved in your care.
  • Speak direct to a manager – should you feel uncomfortable approaching the staff member/s directly, request to speak with their manager.
  • Complete a feedback form – feedback forms are located on all wards and main public areas, or you can request a form from a staff member.
  • Present in person – ask at the main reception to speak with the Family and Patient Liaison Officer.
  • Telephone us – 9531 8000
  • Email – send us an email – PHC.Feedback@health.wa.gov.au
  • Via post – write a letter to the Peel Health Campus via the address provided below.

We endeavour to follow up all feedback in a timely manner and in accordance with the WA Health Management Policy. All information will be treated in confidence.

Share your story online (Care Opinion)

You can share your experience publicly and anonymously via Care Opinion (external site). Care Opinion is a social media platform where you can provide feedback about your experience, or the experience of someone you know, with one of our hospitals or our health service.

Anyone can use Care Opinion. By sharing your story on Care Opinion we listen and respond in ‘real time’ to your experiences and use the feedback to improve the quality of our services.

External support or advocacy

If you are not satisfied with the complaint outcome or resolution provided by Peel Health Campus and need further support or advocacy, contact the:
  • Health and Disability Services Complaints Office (HADSCO) (external site) by phoning 6551 7600 or freecall 1800 813 583.
  • Health Consumers Council (external site) by phoning 1800 620 780 or emailing the Council.
